Thank you for choosing Mountainside Mail. The easiest way to sign up for a mailbox is to bring your two forms of ID and come see our friendly staff in Sequim. However, if you are unable to visit us you may following the steps below to sign up online:

STEP 1
Download and fill out these 2 forms.

1583 Form
Mailbox Rental Agreement
STEP 2
The USPS 1583 form must be notarized and 2 forms of identification must be copied.
Acceptable forms of ID are listed on the USPS 1583 form boxes 8e and 9g. Please see the back of the form (2nd page) for instructions on filling it out. The list of acceptable ID's per the USPS is found here.
STEP 3
Once you have the forms filled out and notarized, please mail the original forms to:

To get your mailbox set up more quickly, you may:

Please note: We will still need the original forms sent to us.
STEP 4
Payment - Package Receiving mailboxes have a $5 set-up fee and will require a credit card on file. Items received will be charged when they arrive. When we receive your paperwork, we will call you to get a credit card set-up in our system.
