Thank you for choosing Mountainside Mail. The easiest way to sign up for a mailbox is to bring your two forms of ID and come see our friendly staff in Sequim. However, if you are unable to visit us you may following the steps below to sign up online:

The USPS 1583 form must be notarized and 2 forms of identification must be copied and sent with your documents.
See boxes 8e and 9g on the USPS 1583 form for proper ID's and follow the directions on the back (2nd page) of the document. This list of acceptable ID's per the USPS is located here.
If you'd like to add scanning or forwarding services to your mailbox please fill out the following form and send along with the other documents:
Once you have the forms filled out and notarized, please mail the original forms to:

To get your mailbox set up more quickly, you may:
Scan and email them to firstname.lastname@example.org or
Fax them to 360 477-4250
Please note: We will still need the original forms sent to us.
Payment - all mailbox rentals will require a credit card on file. After we receive your paperwork for your mailbox, we will call and set-up your desired term and put a credit on file. We will automatically renew your mailbox for the original term on your renewal date unless we receive notice within 7 days of that date. At that time you can change terms or let us know if you'd like to close your mailbox.
*Any scanning and forwarding fees will be added to your account and we will automatically process your credit card for those fees at the end of the month. You will receive a statement on the last day of the month and we will charge the cards on the first.
